Summary:

A 52-week post-graduate educational training program under the direction of an advanced pharmacy preceptor(s).

Responsibilities:

*Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
*Develops knowledge, skills, and experience to become an advanced pharmacy clinician who manages and improves the medication use process ensuring safe, appropriate, and cost-effective therapies for patients.
*Develops and expands upon the knowledge and skills required to provide evidence based, patient-centered medication therapy management within interdisciplinary teams.
*Develops leadership skill to effect change within the pharmacy clinical and operation sections of acute care hospitals.
*Expands upon and improves the pharmacotherapeutic knowledge and communication skills required to provide medication, therapeutics, and practice related education.
*Develops the knowledge and skills to utilize medical informatics by utilizing the informatics tool available at CHRISTUS.
*Assumes roles and responsibilities also listed in Clinical Pharmacist I job description.

Requirements:

  • Graduate from an ACPE accredited Doctor of Pharmacy Program required
  • Must be licensed or eligible for pharmacist licensure within 90 days from first day of employment unless a formal program extension is approved for extenuating circumstances, as guided per ASHP.
  • BLS and either ACLS or PALS (if pediatric practice) within 180 days of start date.
