Job Summary:The Home and Community andBased Services (HCS) Program services people diagnosed with intellectual and/ordevelopmental disabilities who live in a variety of non-institutional settings.Services are designed to meet the person's needs so they may remain in thecommunity and have opportunities to participate in their community to themaximum extent possible.

As an HCS Service Coordinator you willmonitor services and supports for individuals diagnosed with intellectualand/or developmental disabilities (IDD). In addition to IDD, many individualsyou will serve will have mental health and/or medical diagnoses. Individualsserved range in age from very young children to the geriatric population. You will have a designated caseload of individuals for whom you will work withto help improve their quality of life. You will provide these services inthe community, including their place of residence, schools, day programming,etc. The service coordinator role is a dynamic position that entailsworking with the individuals and their support systems, facilitating meetings,crisis prevention and advocacy. As their service coordinator, you willnot only monitor their HCS Services, but also help support individuals inleading meaningful and fulfilling lives.

Responsibilities and Duties

  • Monitor progress, service delivery and satisfaction of services
  • Facilitate meetings amongst the service planning team
  • Complete and monitor individualized care plans
  • Identify and leverage clients own strengths to help them achieve their goals
  • Maintain documentation of all client contacts within required timelines and in compliance with program requirements
  • Develop working relationships with community HCS providers
  • Assist individuals in finding HCS placements
  • Maintain a caseload of approximately 40 clients
  • Provide support to individuals in accessing appropriate services through communication within Integral Care, community partners, service providers and other relevant agencies.
  • Travel locally, daily, for client visits
  • Participate in inter-agency collaborations to problem solve client needs and barriers to progress
  • Utilize electronic health records, Microsoft Word/Outlook and other applications.

  • Graduated from an accreditedcollege or university with a Bachelor'sdegree with a major specialization in Human/Behavioral/Social Services orrelated field.



  • Center certification to performservice coordination (Training will be provided during the onboarding processto obtain this certification.)
  • Current and valid Texasdriver's license and a clean driving record.

Hours: Monday -Friday, 8am-5pm withsome with some flexibility.

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at$38,479-$40,739 based on education, but internal applicants are subject toIntegral Care's QMHP career ladder.
